Scholarship Application Instructions – READ CAREFULLY!!! The application will open in another window and is a Fill-in PDF DO NOT ATTEMPT TO FILL OUT THE APPLICATION ONLINE. YOU MUST DOWNLOAD TO YOUR COMPUTER TO FILL IT OUT!!!!!!!! Graded areas will have a % indicator. The easiest way to complete the application is to type in spaces provided after downloading. HOWEVER, if you choose to fill out as hard copy, you MUST print everything except signature Areas on the application are as follows and are mandatory: First Name, Last Name GPA College Major Consideration for Scholarship (one short paragraph) Community Service (if any, hours, and for whom) Check the box if you plan on working for up to one year after graduation Check the box to verify that you have included your 1-page essay to accompany application Check the box to verify that you will abide by Instructions, Regulations & Restrictions Check the Mission box ONLY if you plan on an LDS Mission after graduation Signature from Mr. Ben Cox, Scholarship Counselor, NSH Student signature in box provided – If typed it will appear as script font and will be your electronic signature Date box – drop down arrow right side of box will show calendar to select date you completed the application There are TWO buttons in lower right side – one is for printing completed application and the other (CAUTION) is for RESETTING the entire form and you have to start over. If mandatory areas are not filled in (except as noted), your application will be rejected!!!  Submission information is provided at the bottom of each application.
